We chat with you to identify your health concerns and your current care team
Assign your coordinator
We assign you to a care coordinator who will be your main point of contact
Collaborate Openly
We will gather information and, where possible, arrange team meetings with you and your care team to review your treatment plan
Review and progress
We then check on your progress with your plan, keep your team updated, and update your plan as needed
