What We Offer

When care involves a lot of people Most of us see more than one practitioner — a GP, specialists, allied health, sometimes a hospital team. They often work from different clinics, each holding their own slice of your information, and they don't always talk to each other. That leaves you (or a family member) doing the remembering, repeating, and chasing.

Care coordination takes that load off you. We help gather your health information into one place, keep everyone informed as things change, and make sure your care actually joins up.

Our Process

Plan with Purpose

We chat with you to identify your health concerns and your current care team

Assign your coordinator

We assign you to a care coordinator who will be your main point of contact

Collaborate Openly

We will gather information and, where possible, arrange team meetings with you and your care team to review your treatment plan

Review and progress

We then check on your progress with your plan, keep your team updated, and update your plan as needed
